class="am-article-wrap">

Inositol is a vitamin-like substance involved in transmitting insulin and hormone signals inside cells. It is increasingly discussed by gynecologists and endocrinologists, yet many women still are not entirely sure what it is, how it works, and when taking it may actually be useful.

In this article, we explain how inositol works in the female body, which conditions have the strongest research support, and where the evidence remains limited. We will also look specifically at PCOS, menstrual cycle regularity, and whether inositol can affect how women feel during their periods.

Key Points

Inositol is naturally involved in transmitting insulin, FSH, and LH signals inside cells. The body produces it on its own, while some is also obtained from food.

Inositol has been studied most extensively in polycystic ovary syndrome (PCOS). It may help make menstrual cycles more regular and improve certain hormonal and metabolic markers, although the overall quality of evidence is still limited.

Inositol is not a painkiller. Its main potential role in PCOS is associated with restoring a more regular and predictable menstrual cycle. Some women may also notice less menstrual discomfort as cycle regularity improves.

By potentially reducing androgen levels, inositol may gradually improve acne and excessive hair growth in women with PCOS. There is also research suggesting that myo-inositol during pregnancy may reduce the risk of gestational diabetes, although there is not enough evidence to recommend it routinely to all pregnant women.

Inositol is generally well tolerated. Higher doses may cause mild gastrointestinal side effects. It is advisable to discuss supplementation with a doctor before starting it.

What Is Inositol?

Inositol is a vitamin-like substance chemically similar to glucose. It was previously classified as a B-group vitamin and was sometimes called “vitamin B8.” Today, it is not considered a true vitamin because the human body is able to produce inositol on its own.

According to different estimates, the body produces approximately 2–4 g of inositol per day, mainly in the kidneys and liver, while around 1 g may come from food.

There are nine forms of inositol. Two are particularly relevant to women’s health:

Myo-inositol (MI) is the most common form found in the human body and in food. This is where the prefix “myo-,” meaning “muscle,” comes from, as seen in words such as “myocardium.”

The word “chiro” comes from the Greek word for “hand.” This form of inositol exists in mirror-image molecular configurations that can be compared to the left and right hands.

Inositol is approximately half as sweet as regular sugar, so inositol powder dissolved in water has a mildly sweet taste.

Myo-inositol used in dietary supplements is often produced from corn and rice bran, while D-chiro-inositol may be derived from raw materials obtained from the carob tree.

How Inositol Works in the Female Body

Inositol participates in the function of so-called secondary messengers. In simplified terms, a hormone binds to a receptor on the surface of a cell, while inositol-containing compounds help transmit that signal inside the cell. If this signaling process is impaired, the cell may respond less effectively to the hormone.

Step 1. Insulin Sensitivity

Many women with PCOS have insulin resistance, meaning their cells respond less effectively to insulin. As a result, the pancreas needs to produce more insulin to maintain normal glucose levels.

Inositol is involved in transmitting insulin signals inside the cell. For this reason, it has been studied as a substance that may improve tissue sensitivity to insulin.

Step 2. Lower Insulin, Lower Androgens

High insulin levels can stimulate the ovaries to produce more testosterone. At the same time, production of sex hormone-binding globulin (SHBG) in the liver may decrease.

When insulin sensitivity improves and insulin levels decrease, the concentration of free androgens may also decline.

Step 3. Follicle Maturation and Ovulation

In the ovaries, myo-inositol is involved in transmitting the FSH signal required for normal follicle maturation.

When hormonal regulation functions more effectively, the likelihood of complete follicle maturation and ovulation may improve, which can contribute to a more regular menstrual cycle.

Step 4. A More Regular Menstrual Cycle

If ovulation is restored, the normal sequence of menstrual cycle phases may gradually become more regular.

For this reason, the main effect of inositol studied in women with PCOS is not pain relief during menstruation but rather improvement in cycle regularity.

Researchers also discuss the so-called “ovarian paradox.” According to this hypothesis, in women with PCOS, insulin may cause increased conversion of myo-inositol into D-chiro-inositol within ovarian tissue. As a result, the amount of myo-inositol involved in FSH signaling may decrease while D-chiro-inositol concentrations rise.

This hypothesis may help explain why not only the dose but also the ratio of the two forms of inositol may be important, and why very high doses of D-chiro-inositol are not always considered optimal for ovarian function.

Inositol for PCOS

PCOS is the condition in which inositol has been studied most extensively. Many women with PCOS also have insulin resistance, which is why improved insulin sensitivity is considered one of the potential mechanisms behind its effects.

In women with PCOS, inositol may:

help make menstrual cycles more regular;

support the restoration of ovulation;

improve insulin sensitivity and certain markers of glucose metabolism;

help reduce androgen levels.

However, the evidence is not completely consistent. For some outcomes, metformin may be more effective. On the other hand, inositol is generally better tolerated and is less likely to cause significant gastrointestinal side effects.

Inositol is not a cure for PCOS and should not replace treatment prescribed by a doctor. It may be considered as an additional option and discussed with a gynecologist or endocrinologist, particularly when metformin is poorly tolerated.

Inositol and the Menstrual Cycle: Regularity and Pain

The effect of inositol on menstrual cycle regularity has been studied mainly in women with PCOS and insulin resistance. Its direct effect on menstrual pain is much less well researched.

Cycle Regularity

Irregular, infrequent, or absent periods are common symptoms of PCOS. With regular inositol supplementation, some women may experience a more predictable menstrual cycle.

Some studies have also reported changes in menstrual flow. In most research, results were assessed after at least three months of regular supplementation.

Menstrual Pain

The mechanism behind typical menstrual pain is not directly related to inositol.

Before menstruation, the lining of the uterus produces increased amounts of prostaglandins, substances that intensify uterine contractions and may cause pain. This is why nonsteroidal anti-inflammatory drugs such as ibuprofen are commonly used for painful periods: they reduce prostaglandin production.

There is currently no convincing evidence that inositol can make periods pain-free in otherwise healthy women.

Any improvement in discomfort is more likely to be indirect:

in women with PCOS, menstrual symptoms may improve as the menstrual cycle becomes more regular;

in endometriosis, one of the common causes of significant menstrual pain, D-chiro-inositol is still being investigated.

If periods are accompanied by severe pain, supplements should not be used as the only approach. Significant menstrual pain may be associated with conditions such as endometriosis, uterine fibroids, or inflammatory disorders. More information is provided in the section “When to See a Doctor.”

Inositol for PMS and PMDD

Premenstrual dysphoric disorder (PMDD) is a severe form of premenstrual syndrome in which mood may worsen significantly before menstruation. Symptoms can include pronounced irritability, anxiety, mood changes, and other emotional symptoms.

Myo-inositol is involved in serotonin signaling, which is why researchers have also explored its possible use in PMDD.

Research findings remain inconsistent. Some studies have reported improvement, while others found no meaningful difference between inositol and placebo.

For this reason, inositol cannot currently be recommended as an established treatment for PMS or PMDD. If symptoms significantly interfere with everyday life, it is advisable to consult a gynecologist or psychiatrist, as there are treatment options with stronger supporting evidence.

Inositol During Pregnancy and Gestational Diabetes

Gestational diabetes is a disorder of glucose metabolism first diagnosed during pregnancy. Because myo-inositol is involved in insulin signaling, researchers have studied whether it may help prevent this condition.

According to available studies, myo-inositol supplementation during pregnancy may reduce the risk of gestational diabetes, hypertensive complications, and premature birth. Some studies found the effect to be more noticeable in women who were overweight.

However, the available evidence is still insufficient to recommend inositol routinely to all pregnant women as a standard preventive measure.

During pregnancy, any supplements, including inositol, should be used only after consultation with an obstetrician or gynecologist.

Acne, Excess Hair Growth and Other Areas of Research

Skin and Hair

Acne and excessive hair growth in women with PCOS are often associated with elevated androgen levels.

If testosterone and other androgen levels decrease while taking inositol, some women may gradually notice an improvement in skin condition and slower growth of unwanted hair.

This effect develops gradually and may take several months to become noticeable. For pronounced hirsutism, available evidence suggests that metformin may be more effective.

What Else Is Being Studied?

Inositol is also being studied in combination with selenium for certain autoimmune thyroid conditions and for supporting metabolic health after menopause.

These areas have been studied much less extensively, so self-supplementation specifically for these purposes is premature.

Which Foods Contain Inositol?

Myo-inositol occurs naturally in many foods, particularly plant-based foods.

Sources include citrus fruits such as oranges and grapefruit, melon, legumes such as beans, chickpeas and lentils, whole grains, nuts and seeds. Small amounts of inositol are also found in meat.

A typical diet provides approximately 1 g of inositol per day. This is considerably less than the amounts used in many studies involving PCOS and pregnancy, where doses of 2–4 g per day were commonly studied.

Obtaining these amounts from food alone would be difficult.

How to Take Inositol

The optimal dose and duration of supplementation should be discussed with a doctor. The information below reflects commonly studied regimens rather than an individual treatment recommendation.

Dosage. One of the most widely studied regimens is 4 g of myo-inositol per day, divided into two doses of 2 g in the morning and 2 g in the evening.

More is not necessarily better. Increasing the dose does not automatically increase the effect. At doses of around 12 g per day, nausea, bloating and other gastrointestinal side effects become more common.

Form used in PCOS. A combination of myo-inositol and D-chiro-inositol in a ratio of 40:1 is commonly used, reflecting their approximate natural ratio in the blood.

Powder or capsules. Inositol powder is usually dissolved in water. It is also available in tablets and capsules. Some manufacturers of softgel products claim increased bioavailability, meaning the amount of active ingredient may vary between different preparations.

Duration. In clinical studies, changes in cycle regularity and hormonal markers were generally assessed after at least three months of supplementation.

Myo-inositol is also frequently combined with folic acid.
